Evidence of Insurability (EOI) Process for Voluntary Life Insurance

Employees who want to purchase voluntary life coverage for the first time or for employees experiencing a qualifying life event who want to make changes to their current elections, you will need to complete the Evidence of Insurability (EOI) process and obtain approval from The Hartford.

 

No action is required if you have already elected the voluntary life insurance coverage and have been approved by The Hartford.

How to Enroll?

 

  1. Log into PlanSource and select voluntary life insurance available through The Hartford

  2. If elected voluntary life insurance amount is higher than the Guaranteed Issue amount, you will be required to fill out a Personal Health Application to get approval

  3. After finishing the enrollment process, a popup will be displayed to let you know that you have to complete a Personal Health Application by clicking “Complete Application” button which will direct you to The Harford portal

  4. You will then complete the application on The Hartford portal


When to expect a response from The Hartford?
 

Once you complete the process, you will receive an immediate response of Approved, Declined, or Pending. If you receive a pending status, then a Medical Underwriter will need to review your information further or request additional information if needed. This may also include obtaining blood/urine testing. The Hartford will attempt to reach out twice, and if there is no action taken, your EOI file will be closed after 60 days.
 

What is your premium payment when EOI approval is pending?
 

When your EOI approval is pending, PlanSource/The Harford will calculate the premiums based on the approved amount only. When the EOI gets approved, the system will update the premium amount accordingly.
